The broadcast sheet is a 1969 form, but the printout looks like it follows the 70 pattern. For example, there is no "6" under the N96 field on line 9 of the sheet, but there is a "6" on line 10, in the radio section, where you would find the engine accessories section on a 1970 form.

I did not notice this originally as I have copied the info above from a member on another forum. Just wanted to share the knowledge over here as well.
